Updated Supplement to Application for Indexed Universal Life (IUL) Products
The ‘Supplement to Application for Indexed UL’ is being updated for all Indexed Universal Life (IUL) products as follows:

The form is being updated in all states except for: California, Delaware, and Florida.
The prior version of the form will no longer be accepted on applications signed June 28, 2021 and after.
The new version of the form has two sections to complete index selections. Which section should be completed is based on the product type. The reason for the two sections is due to the fact that there are different index choices available on the Builder Plus IUL® 3 compared to our other currently marketed IUL products.
SimpleSubmit® and FormsFactory® were updated with the new form on May 22, 2021.
