Revised WA HIV Consent Form – Effective 10/18/21
Effective Monday, October 18, 2021, the Washington HIV Testing Information Statement and Consent form
(ORD 88624-2021_WA) will be updated in response to a regulatory alert issued by the state of Washington.
The Washington HIV Consent form (ORD 88624-2021_WA) has been revised as follows:
- Renaming of the form to Notice and Consent for Blood-Borne Pathogen Testing.
- Inclusion of references to “blood-borne pathogens.”
- Inclusion of description of testing done on bodily fluids.
- Revision of description of how positive and indeterminate results are handled.
- Updating the revision date to 2021.
The revised Washington Notice and Consent for Blood-Borne Pathogen Testing form will be available on the
PruXpress website effective Monday, October 18, 2021. For iGo/eApp, iPipeline will be updated at a future date.
