Revised Michigan HIV Consent Form – Effective Monday, 08/15/22

Effective Monday, August 15, 2022, the Michigan Notice and Consent for AIDS Virus (HIV) Antibody/Antigen Form (ORD 88624 0208_MI) will be updated to include the state-required booklet What You Need to Know About HIV Testing.

• The What You Need to Know About HIV Testing booklet must be left with the proposed insured. These pages will not be returned to the home office.
• The revision date of the Notice and Consent for AIDS Virus (HIV) Antibody/Antigen will remain 02/2008.

The examiner provides the HIV Consent form (ORD 88624) to the proposed insured at the time of the exam. This form is to be completed by the financial professional only when age and amount requirements are externally ordered.

The revised form(s) will be available on the PruXpress website effective Monday, August 15, 2022.
