Removal of Term Elite from the Individual Application Effective 11/22/21

Effective Monday, November 22, 2021, the Application for Life Insurance (ORD 96200) and PruXpress QuickForm (ORD 113034) will be revised to remove the retired Term Elite product. The forms will carry a 2021 revision date.

Application for Life Insurance Changes:

• Page 1, section B – Plan of Insurance: remove Term Elite product.
• The forms will carry a revision date of 2021.
• The generic and all state-specific applications are impacted.

PruXpress QuickForm Changes:

• Page 1, section C – Plan of Insurance: remove Term Elite product.
• The forms will carry a revision date of 10/2021.
• The generic and all state-specific applications are impacted.

For all submission methods, the revised form is required with applications with a Part 1 date of November 22, 2021, and later. The previous revision will continue to be accepted on applications with a Part 1 date up to and including November 21, 2021.

The revised form(s) will be available on the PruXpress website effective Monday, November 22, 2021.
