New Premium Deposit Account Agreement – Effective 08/16/21

Effective Monday, August 16, 2021, a new Premium Deposit Account Agreement form (ORD 115811) will be introduced for use with Premium Deposit Account (PDA) 2.0. The form will carry a 2021 revision date. Please refer to the Product Availability Chart for product and rider availability by state. The form must be submitted when applying for PDA 2.0. The revised form(s) will be available on the PruXpress website effective Monday, August 16, 2021.
