Illinois Secondary Addressee – Effective 01/01/22
Effective January 1, 2022, the state of Illinois adopted regulations allowing policyholders to identify a secondary addressee to receive a notice of cancellation for nonpayment of premium; as a result, a new generic Secondary Addressee Notice (ORD 112674 2021) has been created and will be available for use.
Secondary Addressee Notice
The generic Secondary Addressee Notice form (ORD 112674 2021) can also be submitted with the following states:
- California
- Connecticut
- Idaho
- Illinois
- Maine
- Montana
- New Jersey
- Rhode Island
- Utah
- Vermont
The revised form(s) will be available on the PruXpress website effective December 30, 2021.
