Required Plan Onboarding Forms Now Available in eApp

Earlier this year, we introduced enhancements to administration for plans using life insurance. As part of these updates, two forms were introduced as requirements for plan onboarding – the Key Decision Maker form (DD9699) and the Business Plan Submission Checklist (DD849R). 

We’re pleased to let you know both forms are now included in iPipeline’s eApp for all available products, making it easy for you to obtain them. 

• If the policy owner and the key decision maker for the plan are the same person, the forms will be included together in the eSign application packet.  
• If the owner and key decision maker are different, the Key Decision Maker form will be included in the agent instruction packet to be either ink signed or electronically signed using an approved e-signature vendor.
