COD and Service Forms Update

We’ve updated the forms listed below in most states. The new version date reflects the date the form was filed for approval with the states.

  • Internal Replacement Disclosure Form: 16902, Ver. 05/18
  • Trust Verification Form: 16541, Ver. 05/20
  • Certificates of Disclosure (CODs): Ver. 06/21 (Form number varies by product and state). Affected products include:
  • Athene AccumulatorSM
    • Athene AgilitySM
    • Athene AscentSM Pro
    • Athene AscentSM Pro 10 Bonus

The revised forms are available now on Athene Connect and should be used immediately. Applications with outdated or missing forms received after market close (3 p.m. CT) on July 2, 2021 will be considered “not in good order.” Be sure to use up-to-date materials when offering Athene annuities.
