New Procedures for Non-Individual Contract Owners

We are making it easier for you to submit new business. As of Jan. 4, 2022, we have new procedures for non-individual contract owners. Below, you will find links to two new forms and important information to assist you with submitting this type of business moving forward.

Certification of Trust (form 014260)

Certification of Entity Ownership (form 014385)

Trusts

Please submit our new Certification of Trust form when a trust is purchasing an annuity contract. This new form is replacing our previous Certification of Trust and Trust Statement of Understanding forms. The current trustees of the trust must complete all information on this new form. We will no longer require pages of the actual trust agreement.

Annuitants: The grantor of the trust must be the annuitant if either (1) the trust qualifies as a grantor trust (as defined by the Internal Revenue Code); or (2) the grantor’s Social Security number is being used for the trust’s Taxpayer Identification Number. The grantor’s spouse may be a joint annuitant if permitted by the terms of the trust.

All other non-individual owners

Please submit our new Certification of Entity Ownership form when a non-individual, other than a trust, is purchasing a new annuity contract. This form is replacing our previous Corporate Certification form. An officer of the entity must complete all information on this new form. We will no longer require additional documentation from the entity.

Annuitants: The annuitant must be an officer of the entity purchasing the contract. We do not permit joint annuitants on contracts owned by non-individuals, other than trusts.

As a reminder, not all non-individual owners qualify for income tax deferral on annuities. Please be sure your client is consulting a qualified tax advisor.
