Revised DC HIV Consent Form – Effective Monday, 12/11/2023

Effective Monday, December 11, 2023, the Washington, D.C., Department of Insurance address will be updated on the DC state specific Notice and Consent for AIDS Virus (HIV) Antibody/Antigen form (ORD 88624 2016_DC_2023).

The revised form is required with a Part 1 date of 12/11/2023 and later. Older versions of the form will continue to be accepted until Monday, February 12, 2024. The updated form will be available on PruXpress on Monday, December 11, 2023.
